Case 20
A great dentist is one that knows when to refer
(Fig 20A)
A dentist who attended one of the author’s live
courses called asking for help for a separated
instrument. He acknowledged that he had jumped
the gun too fast wanting to get the file to length
before adequate coronal flaring that resulted in file
separation.
(Fig 20B)
Without a microscope, the dentist tried to remove
the separated file that was lodged at the orifice, but
without luck. He had considered alternate plans,
including obturating with the instrument in the canal
or convincing the patient to opt for extraction, but
decided to refer to ease his conscience.
(Fig 20C)
(Image not from this case) In the past, silverpoints
were often used as the obturation material,
however, since most canals are not symmetrically
circular, cases that were obturated with
silverpoints often failed due to inadequate fill.
