A female patient in her 30s presented with a pre-op radiograph showing widened

            PDL at the root apex of tooth  15 that is deviated to the distal (as opposed to being at the
            centre of the apex) (Fig 4A). After proper root canal treatment, the obturation radiograph
            revealed a sealed accessory canal at the root location adjacent to the area of widened PDL
            (Fig 4B). The six-months follow-up radiograph was taken at a different angle and shows
            the accessory canals branching out from the main canal at the apical third of the root (Fig

            4C). In addition, the enlarged image of the apical portion revealed multiple radio-opaque
            sealer  puffs  at  the  exits  of  those  accessory  canals  (Fig  4D).  During  warm  vertical
            obturation,  the  sealers  will  fill  the  canals  first,  and  as  the  warmed  gutta  percha  is

            compacted into these spaces, the sealers will be extruded outside the canals. Therefore, a
            densely obturated canal system  will often have accompanying sealer puffs at all canal
            exits.
