Regular follow-ups will reveal the difference between
overextension and overfilling

Improper obturation of tooth 25 resulted in
pain and discomfort for the patient. The patient
expressed difficulty choosing between various
treatment options since the crown was newly
cemented, and she did not want it
disassembled.

The patient was informed that apical surgery
would not be able to address the distal mid-root
lesion and the possible lateral canals at that
location. After discussing the risks, benefits and
alternatives of each treatment option, the
patient agreed to have the crown removed and
complete root canal retreatment.

Two lateral canals are seen densely obturated
after retreatment. A white-in-white dot is also
seen at the exit of the main canal, which
signifies a properly overfilled canal system.
