Sudden disappearance of a main canal indicates the existence of
split canals

The sudden disappearance of the Areas of widened periodontal ligament

An apical radiolucency around the right maxillary second premolar ( 15) was
observed on the pre-operative radiograph (Fig 1A). Tracing the main canal from the
coronal portion apically, the previously clear and distinctive canal suddenly blurs past the
middle third of the root. It is possible that a split in the canal, a curved canal, or a
dilacerated canal exists at that location.
After proper cleaning, shaping, and 3D warm vertical obturation of the root canal
system, the post-obturation radiograph confirmed the split of the main canal into two
separate canals apical to the middle third of the root, and even showed another lateral
canal splitting off one of them (Fig 1B). Additionally, the radiograph showed multiple
white-in-white dots between the main canal and the lateral canal. These represent the
small and narrow accessory canals that have been properly sealed.
