CC: Referred by her periodontist for evaluation of 37. The periodontist had

treatment planned to amputate the distolingual (DL) root of 37 during
periodontal surgery if the tooth is to be retained.
HCC: The patient complained of bad smell and swollen gingiva on the lower
left posterior. She reported that a chunk of filling material broke off
while she was eating several weeks ago, she did not seek dental care at
that time since it was not sensitive and did not bother her. She reported
that the odour is still present even with frequent brushing and mouth
rinses.

O I/O: 37 DL PD 10mm, crack lines visible on the DL crown structure
